DAISY Foundation Overview
Founded in memory of J. Patrick Barnes, the DAISY Foundation is a non-profit established to honor exceptional nursing care. J. Patrick Barnes died at age 33 from complications of Idiopathic Thrombocytopenic Purpura (ITP); DAISY stands for "Diseases Attacking the Immune System." The foundation recognizes nurses who demonstrate extraordinary compassion and skill in patient care. This award program highlights nursing contributions that advance health equity and support underserved populations.

Nomination Criteria
Description: Nominations should explain what sets the nominee apart from other caring nurses or teams. Submissions must demonstrate how the nominee meets at least one of the award criteria related to community commitment, addressing social determinants of health, impact on patient care or the nursing profession, or excellence beyond expected duties. Use concrete examples and supporting materials to document impact and outcomes. Nominations may be submitted for individuals or organizations.

AAMN DAISY Health Equity Award — Individual
Description: This award recognizes an individual nurse who goes above and beyond to address health equity and mitigate social determinants of health. Recipients are those whose actions materially improve access, outcomes, or the health experience for underserved populations. The award emphasizes sustained commitment and measurable impact in the community or clinical setting. Selection prioritizes examples of leadership, advocacy, and compassionate, equitable care.
Eligibility:
- Current full membership in AAMN
- Not a current AAMN Board member
- Not held a Board position within two years prior to application
- Promotes and enhances the image of nursing within the organization, community, and profession
- Promotes health equity within their community
Award Value: Healer’s Touch sculpture and a personalized DAISY award certificate.

AAMN DAISY Health Equity Award — Team
Description: This award recognizes a healthcare organization or team whose collaborative efforts address health equity and support patients and families from historically underserved populations. Teams honored demonstrate organizational commitment to equal access, professional development, and community-focused improvement. Nominations should illustrate teamwork, outcomes, and sustained organizational practices that reduce disparities. Selection values role-model behavior that benefits patients, families, employees, and the broader community.
Eligibility:
- Organization exemplifies AAMN’s mission by ensuring equal access to professional development and improving health
- Members serve as role models of collaboration and teamwork impacting patients, families, employees, and community
- Nomination must be submitted by an organization member

Application Instructions
Description: All submissions and supporting materials must be submitted electronically as a single PDF. Assemble required documents, use the specified file naming convention, and email the consolidated PDF and separate headshot to the administrative address. Incomplete applications or submissions consisting only of a CV will not be reviewed. Follow the listed requirements precisely to ensure consideration.
Required Materials:
- A one-page summary of the achievements that qualify the nominee or team for the award
- A one-page bio with a current high-resolution professional photo (headshot)
- A PDF attachment with supporting materials (e.g., photos, newsletter articles) to substantiate qualifications
Submission Details: Save all materials as one PDF file named "(Your Last name)-Award Application" (for example, Jones-Award Application). Email the PDF file and use the same file name in the subject line to admin@aamn.org. Submit the high-resolution headshot to the same email address.
Important: APPLICATION MUST BE COMPLETED IN ITS ENTIRETY. Attachment of the CV only does not fulfill this requirement and will not be reviewed.
